Ongoing Support for Client and Family
- In-person comfort and companionship
- A safe, non-judgmental space to speak openly about death and dying
- Education about death and grief to help prepare for anticipated life changes
- Planning for therapeutic, personalized hospice space to promote peace and comfort
- Non-medical comfort measures (Reiki, guided visualization, aromatherapy, reading, sound therapy, Gentle Touch massage, crystal healing)
- Education, planning, and support for VSED (Voluntarily Stopping Eating and Drinking) and MAID (Medical Aid in Dying in VT)
- Logistical help (setting up visiting hours, shopping, errands, pet care, child care, transportation, meal preparation, etc)
- Assistance with self-advocacy to insure client's wishes are honored
- Spiritual support honoring client's personal belief system
- Ritual planning
- Coordination and communication with hospice and other clinical providers
- Immediate, urgent problem-solving assistance when dying occurs
- Vigil sitting
- Grief support for family
Advance Directives, Resources, and Referrals
- Guidance with navigating resources and requirements for palliative care, hospice, caregiving, and proxies in Massachusetts
- Provision of resources and documentation for end-of-life decisions
- Assistance preparing Advance Care Directives
- After-death support, notifications, and communication with funeral directors
- Organization of important information that will be needed after death
- Caregiver resources (pet care, maid services, meals)
- Referrals for hospice and palliative care
- Dementia and Alzheimers care resources
- Arranging visits with holistic practitioners (music therapy, massage, pet therapy)
- Resources and referrals for funeral homes and cemeteries
- Resources and referrals for green burials and natural burial containers
- Referrals for grief counselors and support groups
- Resources for religious organizations
Life and Legacy Work
- Assistance with getting affairs in order
- Design and creation of legacy projects, including but not limited to autobiographies, scrapbooks, treasure boxes, ethical wills, legacy letters, photo albums, memorials, audio and video recordings, heirlooms)
- Hands-on, developmentally appropriate support and guidance with working with children and their involvement in the end-of-life process
- Eulogy and obituary writing
